Sam and Dean deal with a witch who is using her powers to terrorize a small town. Once he gets Dean up off the floor, Bobby says that they have to go and leaves to find Sam. He calls them pets and tells them that if they rise up against him, he'll kill them. They go into the apartment, identifying themselves as FBI agents, and the detective in charge explains that Hammond was apparently crushed to death by a car. Castiel's trenchcoat washes up on shore and they figure that he's dead.

The black fluid spreads across the area. A young girl, Annie, is possessed when she drinks from a fountain, and a water pipe explodes in a mechanic's face. Castiel: So just what is going on with Cas, as the season opens? Said Singer, "I think he totally bought into what he was doing at the end of last year. He totally believes in what he is doing and believes that it's the right thing. There's a certain amount of hubris and ego there that's probably misplaced." Meanwhile, Dean talks to Chris at the salon and confirms that the hairdryer couldn't have done what it did.

That episode will see the return of the late Jo Harvelle (Alona Tal), who is called as a witness against Dean. Singer had a ton of praise for Tal, saying, "She's great in the episode. We put some flashbacks in the episode, back to an episode she was in years ago, and she has matured and incredibly grown as an actress. She's just wonderful. I hadn't really worked with her for quite sometime, and I was just blown away by what she did." He looks around and finds coin behind the chair, and calls to tell Sam what he's found. As Dean walks down the street, he notices a bus bench with an advertisement photo of Wendy on it, surrounded by dead plants. A drug dealer is making a deal but is interrupted when a police siren goes off nearby. His customer runs and the drug dealer takes off. Something grabs him and kills him, drilling a hole in his skull.

He's learned from the e-logs that Dewey, Wendy, and Carl were all working on a mall construction project and it fell through. Old Demons: Promises Gamble, "You'll see more of Crowley (Mark Sheppard). Crowley is sort of the ultimate self-interested character. He always finds a way to bend whatever is happening to his own best uses. And as the weather kind of changes, he figures out how to insert himself into the situation. Dean takes Sam out and says that he doesn't see why they have to investigate because the victims are getting what they deserve. Bobby calls in and tells them that the coin is a Romanian ducat dating back to the 15th century. They notice that the plants around a statue of Don are dying, and Dean remembers the plants around Wendy's picture.

Young Sam goes to get a book and sees the girl in the next aisle, looking at him. He then calls Dean and tells him the way to kill the kitsune is to stab it in the heart. Jenny is making cupcakes in her kitchen, unaware that Maggie is using her blood to cast a spell on another photo of Jenny. Jenny samples one of her cupcakes but then realizes that the cupcake is oozing blood from a small beating heart inside of it. She drops it and starts choking, vomiting up blood.

Dean is chained to a chair and Osiris is dressed in full regalia and seated on a throne. He points out that he listened to Dean talking about his guilt at the bar, and then calls out to Sam, who he knows is in the shadows. Maggie comes back and Don talks to her, insisting that none of it was he fault and that Maggie was the one who was never there for him. Edgar and the two swim team boys arrive at the hospital and Dr. Gaines brings them in. He gives them a tour of the hospital and explains how it works, and how no one ever questions it when people die. He has the head nurse and staff administrator tied up.

Sam finishes telling Dean what happens. He doesn't believe that Amy committed the murders but Dean insists that they have to drop her now no matter what she did as a kid. When Dean says that it's sample, Sam points out that nothing in their lives is simple. He prepares to go to a synagogue to find one, but Dean is worried that Osiris will send Jo after him. Sam reminds his brother that Dean wasn't the one that killed Jo and tells him to do what he has to, to protect himself from ghosts. Castiel wakes up and finds himself covered in blood and surrounded by all of the senator's dead aides. The whispering of the Leviathans echoes in his head as Castiel stares around in disbelief.

In response, Sam says that there's nothing Dean could do so he kept it to himself, but Dean says he's giving up and will stick to drinking. However, he discovers an article about Castiel's slaughter at the senator's campaign office. There's security footage that reveals that Castiel smirked as he slaughtered them, and Dean figures there's no way to reach him. He grabs both of them and starts choking them, but Don arrives and brings him down with a magic spell. They thank him and he wonders what it is, and says that his spell will only last for a couple of days. Don then removes one of Maggie's hex coins from beneath their bed, pointing out that they did try to kill her and she still wants revenge. Dean warns that she's a killer and eventually she'll kill again no matter what she claims. Sam asks his brother who the final witness would have been, but Dean lies and says he has no idea.
