Castiel spares Bobby and the Winchesters and goes on his way to set right the wrongs of the world. He passes out and sometime later, pulls out the IV and tries to get out of bed only to discover that he has a cast on his leg. As they do, Castiel says that there's no point if they do it out of fear rather than respect. When Gaines comes out after them, Dean manages to get in just in time and they drive off. Gaines and the head nurse make a call. Dean and Bobby find Sam, who apparently snaps out of his hallucination of Lucifer, and they leave together.

Dean snaps at him briefly but then admits that he's right and vows to do what he can, starting with fixing the Impala. From the Supernatural panel at Comic-Con 2011, we know that Jo, Crowley, Sheriff Jodie Mills, and Death will be returning this season. She died at the age of 10 and they head back to the hotel to do some checking.

The Trial of Dean Winchester: One big episode for Dean will involve him being put on trial by the Egyptian God, Osiris, who Singer notes, "weighs your heavy heart against a feather and if your heart is heavier than the feather, then he does you in. And it's all about carrying guilt and that sort of thing. Not just, 'Do I feel bad?' But, 'Do I feel guilty?' And that's kind of a big turning point episode for [Dean's] arc this year." It turns out that Hammond had accidentally run her over 10 years ago. Dean, drinking heavily, wonders if Hammond was drunk when he ran Elizabeth over. Sam and Dean return to the cemetery and salt and burn Elizabeth's bones. They wonder if there's something they can do for Sam, but Bobby asks Dean how he's doing. Dean insists that he's fine and Bobby points out that Dean was pissed when Sam said the same thing. Bobby isn't convinced and tells Dean that he'll be there when he wants to tell the truth.

Dean stops the Impala just in time and the man, Warren, tries to catch his breath. They take him to the hotel and Warren says that they won't believe what he has to say. Dean stops in at the grocery store and confirms that Sam bought a newspaper. He checks out the day-old copy and spots the icepick killer ad. He finally admits that he was put on trial and sentenced to death for robbing a liquor store and killing the wife and owner in 1981. Warren just got out of prison after a 30-year term, but he woke up in a courtroom in a barn at the apple farm. Sam returns to see Warren, who refuses to go with him. He claims that the people he killed are out there waiting for him. Sam puts him inside of a circle of salt and leaves him with the TV remote, and then sends the symbols to Bobby to look up.

The mechanic, Edgar, meets with the girl, Annie, and they discuss their respective bodies. The Edgar Leviathan says that his host body is skilled in demolitions, and then tells the Annie-Leviathan to get the ones occupying the swim team under control. At the hotel, Warren is watching the TV when it goes out and the lights flicker. He inadvertently breaks the salt circle and then runs out into the hallway. His victims are waiting for him on the stairs and he turns and goes back, only to find them waiting for him. The owner draws a gun and shoots Warren dead.

Osiris tells Sam to leave before he decides him, but Sam insists that his brother should have a lawyer. Intrigued, Osiris agrees and calls three witnesses, ignoring Sam's objections. He tells her that she's had her fun with Wendy and it's time to stop. Maggie blames Don and his ego, and he points out that two FBI agents came by. Edgar congratulates him and says that's exactly what the boss wants, and tells the two swim team boys to take over the bodies. When Gaines asks if Edgar is going to stay, he says he has to check out some other businesses in the area.

Young Sam tells Amy to get out of town on the bus while he takes care of the body. She asks him to come with her so that they can be freaks together, but he says that he can't. Once Dean encloses himself in a salt circle, he tells Jo she can come out now and she emerges from the shadows. Sam breaks into a synagogue and finds a ram's horn. The rabbi comes in and asks why he's there. Castiel wakes up and finds himself covered in blood and surrounded by all of the senator's dead aides. The whispering of the Leviathans echoes in his head as Castiel stares around in disbelief.

Sam goes out into the junkyard alone and preys to Castiel. He insists that Castiel is still one of them deep down and asks him to let them help. He goes back inside and has a drunken Dean turn off his porn, and then takes a drink himself. Castiel appears, covered in blood, and says that he needs their help. The brothers toss Chet in the trunk and call Bobby to say that they'll be making a delivery. As they prepare to leave, Sam asks if Dean listened to the Starks. He talks about how they opened up and that it worked, and that Dean should do the same. He stabs her and apologizes, and then sets her down on the bed. As he turns to go, he sees Jacob in the door, watching him. He then asks Sam why he wasn't on trial, and Sam says that he simply doesn't feel guilty because he paid his dues in Hell.
